HealthMark Group fulfills all patient requests for personal copies at no charge to the patient. Third-party&nbsp;requestors will be sent an invoice before HealthMark releases records. By default, records will be sent to you&nbsp;via email or fax. You can get a hard copy mailed to you by calling HealthMark to specifically request paper copies.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">To Request a Copy of Your Medical Records:&nbsp;<\/h3>\n\n\n\n<ul>\n<li>Register for an account at <a href=\"https:\/\/requestmanager.healthmark-group.com\/register\" target=\"_blank\" rel=\"noreferrer noopener\">https:\/\/requestmanager.healthmark-group.com\/register<\/a>.&nbsp;<\/li>\n\n\n\n<li>Once logged in, select \u201cOpen New Request\u201d from the menu options and enter all required fields. This&nbsp;will provide authorization to HealthMark.&nbsp;<\/li>\n\n\n\n<li>Your medical record request will be processed, and a notification will be sent via mail or email once&nbsp;it is complete and available for download.&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>If you have any questions, please log in to Request Manager for status updates or to chat with support.
