Policy: Shipping of Hearing Aids and Supplies

We encourage patients to receive supplies and hearing devices at the time of their appointment. In the event that a patient is unable to receive products in person, they may request shipment of their purchase. Payment in full must be received for product cost plus shipping fees prior to shipment.

We encourage patient to purchase shipping insurance for all purchases. In the event the patient waives shipping insurance, St Cloud Ear, Nose and Throat Clinic is not liable for loss or damage of their product during shipment. A shipment waiver must be signed by the patient prior to shipping.

Supplies:

The patient is responsible for the shipping costs of all supplies. Payment in full must be made prior to shipping. St Cloud Ear, Nose and Throat Clinic is not liable for loss or damage of supplies during shipment.

Hearing Aids:

The patient will be responsible for the cost of shipping devices, along with the cost of insuring the shipment based upon its value. In the event the patient chooses not to purchase shipping insurance, they must sign a waiver acknowledging St Cloud Ear, Nose and Throat Clinic is not liable for loss or damage of the hearing aids during shipment.

Shipment Waiver:

This waiver releases St. Cloud Ear, Nose and Throat Clinic from all liability relating to loss or damage in the shipment of my hearing aids/supplies. By signing this waiver, I agree to hold St. Cloud Ear, Nose and Throat Clinic entirely free from any liability, including financial responsibility for damage or loss of my hearing aids. I take full responsibility and acknowledge that I have waived the shipping carrier insurance coverage on this shipment. I fully understand and agree to the terms above.
